Summary of Position:
The Medical Adherence Specialist is responsible for the coordination of health care for ASD residents and making appropriate medical service referrals as needed. The Medical Adherence Specialist works under the supervision of the Medical Case Manager Supervisor.
This is a full time position that will require occasional additional hours.
Duties and Responsibilities Summary:
Conduct assessments, acuity levels, care plans and goals with clients.
Conduct medication adherence and treatment adherence counseling.
Monitor medical care adherence, lab reports, and assist with completion of referrals.
Assist with transfer to case management services when client is independent with healthcare.
Qualifications
Sensitivity to culturally, racially and sexually diverse client, staff and volunteer populations is essential. An awareness of AIDS/HIV-related issues is desirable. Two (2) years of nursing or case management/social work experience are desired. The candidate must be able to negotiate stairways and to access emergency alarm equipment immediately. Must be able to assist with the emergency evacuation of medically compromised clients/residents in the event of fire or other emergency situations requiring evacuation of any facility. Writing and organizational skills are required for this position. General office skills are necessary. A valid driver license is required.
Work Hours
Regular office hours are 8:30 a.m. to 5:00 p.m. Monday through Friday
Salary Range
$40,000.00 – $47,000.00 per year
