In order to become a resident of AIDS Services of Dallas, an intake application must be completed with the Intake Specialist. Placement is based on homeless status and availability of units.

Due to COVID, intakes not done at The Bridge are by appointment only.

*If an individual is physically unable to come in to complete an intake application, we can offer home or inpatient visits on a case by case basis (Dallas County only).
Intake Documents:
At time of intake, the following documents are required:

- State issued identification card
- Social Security Card
- Birth Certificate
- Current proof of income
- Current proof of health insurance
- Letter of Diagnosis
- Current proof of homelessness
- Emergency Contact information (No Case Managers. MUST be family, friends or associates)
*NOTE: Some applicants may not have income, health insurance or homeless letter and can still apply.
If you do not have the required documents you can initiate the process but it will not be complete until all documents have been received.
