CONTRACT TERM: May 1, 2023 – July 31, 2026

PRIMARY FUNCTION

The Community Housing Engagement Specialist plays a vital role in our Emergency Housing Assistance Program, focusing on providing support and assistance to individuals in the LGBTQ+ community who are at risk of or experiencing homelessness due to HIV/AIDS. This full-time position is responsible for engaging with clients, conducting assessments, and developing individualized plans to address barriers and challenges they face. The Community Housing Engagement Specialist will work closely with clients to stabilize their wellness and health, develop skills, and provide education and training related to income and employment.

DUTIES AND RESPONSIBILITIES

  • Conduct intake assessments with clients, confirming their HIV/AIDS diagnosis and identifying barriers and challenges such as gender and sexuality, physical or mental illness, substance use, or underemployment.
  • Develop individualized plans for clients to address their specific needs, including housing placement, skill development, education, and income and employment training.
  • Assist clients in locating eligible housing options and provide support throughout the housing placement process.
  • Collaborate with community partners to ensure access to resources and services, such as healthcare and vocational support.
  • Maintain accurate and up-to-date client records, including progress notes, assessments, and case management documentation.
  • Participate in regular team meetings and case conferences to discuss client progress and coordinate services effectively.
  • Provide ongoing support, advocacy, and crisis intervention as needed.

QUALIFICATIONS

  • Bachelor’s degree in social work, psychology, or a related field.
  • Experience working with individuals from the LGBTQ+ community and knowledge of the unique challenges they may face.
  • Strong understanding of HIV/AIDS issues, housing instability, and social determinants of health.
  • Excellent communication and interpersonal skills, with the ability to build rapport and establish trusting relationships with clients.
  • Ability to work collaboratively with a diverse team and community partners.
  • Strong organizational and time management skills, with the ability to prioritize tasks effectively.
  • Proficiency in documentation and record-keeping.
  • Knowledge of local community resources and service providers.

WORK HOURS:
Regular office hours are 8:30 a.m. to 5:00 p.m. Monday through Friday.

SALARY RANGE:

$48,000.00 – $49,500.00 per year
